Scheduling Appointments

Please call Central Scheduling at 517-364-5255.

We need the following pieces of information when scheduling an initial appointment:

  1. Prescription information
  2. Insurance card
  3. If Auto or Workers Compensation case
    • Case manager phone #
    • Employer
    • Authorization or claim #
    • Date of injury and location
  4. Please have your calendar available to determine best availability for appointment.

We will do our best to reschedule your missed appointments. Due to patient scheduling, it is important that you arrive on time for your appointment to allow us to provide full treatment to you. Your appointment time is very important to us and we will give you our undivided attention. We request your cooperation by being on time for your appointments and by having any forms, authorizations, identification, insurance information and diagnostic reports that may be necessary for your visit with us.

Prescriptions for therapy: By Michigan law, we must have a current prescription that has been written within 30 days of your initial visit for occupational, physical or speech therapy. The prescription should include diagnosis, frequency and duration of treatment. The prescription must be from a physician (MD/DO), physician’s assistant, or dentist.

In addition, we will request to make a copy of a pictured ID.

CANCELLATIONS: We request a 24-hour notice from patients who find it necessary to cancel a scheduled appointment with one of our Physicians or Therapists. If you cancel and/or do not show for more than three appointments in a row, it is within the provider’s discretion that you may be discharged from services.

INSURANCE QUESTIONS: To inquire about insurance participation, surgery benefits, co-pays and deductible, please contact your insurance carrier. For other insurance questions, our team would be happy to assist you.
